1. Introduction
At Özel Curavita Evde Bakım Merkezi (Curavita private home-care center), we regard respect for the rights of every patient to whom we provide home healthcare services as the fundamental principle of our service. This document has been prepared in plain language on the basis of the principles set out in the Ministry of Health's Regulation on Patient Rights, adapted to the specific nature of home healthcare.
This document is provided for information purposes. Your rights are determined by the relevant legislation; this document does not limit the rights you have under that legislation. The rights of patients who are minors or who lack decision-making capacity are exercised by their parents, guardians or legal representatives, within the framework of the legislation.
2. Access to Services in General
- You have the right to benefit from healthcare services fairly and equitably, without discrimination on grounds such as race, language, religion, denomination, gender, political opinion, philosophical belief, or economic and social status
- You may ask for your care to be planned and provided in line with medical requirements and the rules of the profession
- You have the right to ask that priorities in service planning be set fairly and according to medical need
3. Requesting Information and Being Informed
- You may request information about the services our center provides, how services are planned and how you can reach us
- You have the right to be informed about your health condition, the procedures to be performed, their possible benefits and risks, any alternatives, and the possible consequences of refusing a procedure
- Information is given in language you can understand, avoiding medical terminology as far as possible
- If you wish, you may state that you do not want to be informed about your health condition, or specify which of your relatives the information may be shared with
- You may review your health records, request a copy, and ask for incomplete or inaccurate information to be corrected
4. Knowing and Choosing the Provider and Staff
- You have the right to know the organization you receive services from; our center's details and contact channels are listed in the "Right to Submit Applications and Complaints" section of this document
- You have the right to choose which organization provides your home healthcare services and to change it whenever you wish
- You have the right to know the identity, title and role of the healthcare workers who come to your home; you may ask for this information during the visit
- To the extent our center's resources allow, you may ask to choose or change the physician or other healthcare workers responsible for your care; your request will be assessed with due regard to the continuity and planning of the service
5. Privacy and Confidentiality of Personal Data
- You have the right to ask that your privacy be respected during examinations and care procedures; you may ask for someone close to you to be present, or for the procedure to be carried out only by the healthcare workers required for it
- Information about your health condition is kept confidential; it is not shared with third parties except where permitted or required by law, or with your consent
- Information about your family and private life that becomes known during visits is also kept confidential
- Your personal data is processed in accordance with the Personal Data Protection Law No. 6698; detailed information can be found in the KVKK Privacy Notice
6. Consent to Medical Intervention, Refusing and Stopping Treatment
- As a rule, medical interventions are performed with your consent after you have been adequately informed
- For patients who are minors or who lack decision-making capacity, consent is given by the parent, guardian or legal representative; the patient also takes part in the decision-making process as far as possible
- This is without prejudice to exceptional cases provided for in the legislation, such as life-threatening emergencies
- Except where required by law, you have the right to refuse treatment recommended to you or to ask for treatment already in progress to be stopped
- In this case, the possible consequences of not receiving the treatment are explained to you or your legal representative, and your decision is recorded in writing; you are responsible for the consequences of a decision you make despite having been informed
7. Dignity and Comfort
- You have the right to receive services with respect, care and courtesy at every stage; your individuality, values and beliefs are respected
- Because the service is provided in your home, visits are scheduled as far as possible at times that suit the daily life of you and your family, and your household routine is respected
- Within the limits of medical possibilities, due care is taken to relieve your pain and discomfort
8. Safety
- You have the right to expect and request safety while receiving services
- During care, the rules on hygiene, infection control and the safe use of medicines and supplies are followed

- If you notice anything that puts your safety at risk, you can report it to us immediately; in an emergency, call 112
9. Fulfilling Religious Obligations
So that you can perform the prayers and religious obligations required by your faith, your preferences are taken into account when visit and care times are planned, to the extent that this is medically appropriate. Your beliefs and your preferences in this regard are respected during care.
10. Right to Submit Applications and Complaints
If you believe that your rights have been violated, or if you have a complaint or suggestion about our service, we kindly ask you to contact us first. Your application will be reviewed and we will get back to you as soon as possible.

11. Responsibilities of Patients and Their Families
So that the service can be provided safely and effectively, patients and their families are expected to:
- Provide accurate and complete information about the patient's health condition, the medications being taken and past illnesses
- Follow the recommended treatment and care plan, and inform the healthcare worker whenever it cannot be followed
- Have prescriptions, medical reports and test results ready for the visit
- Notify us of appointment changes and cancellations in good time
- Provide a suitable environment so that care can be delivered safely
- Treat healthcare workers with respect and never resort to verbal or physical violence
- Not make requests that are contrary to the legislation or the rules of the profession
- Call 112 without delay in an emergency
